A group of community leaders within the Grand Valley area borne out of the SDoH conversations between the Accountable Health Communities Model (AHCM) staff at Rocky Mtn. Health Plans, the Community Resource Network (CRN) staff at Quality Health Network (QHN) and Hilltop Family Resources staff.
Our desire, ultimately, is to see that all organizations, networks and work groups in the community are aware of the tools available through the utilization of the CRN, 211 and AHCM social needs screening data. (While AHCM is a federal grant project that ends 4.30.22, Quality Health Network will continue to house the social needs screening data after the project is over.)
