This network was created to be the parent network for Mesa County that all other networks can connect to/under. The goal of this mapping is to see not only the organizations but the various networks and work groups that exist in the community that are in some way addressing social determinants of health - whether it's providers screening their patients/clients, community resource providers helping people connect with resources, networks/work groups meeting regularly to make sure gaps are being addressed/resources are being shared/knowledge shared/etc., other organizations or groups who provide services or data or other helps to address SDoH in the community.
Hopefully this will also help identify gaps as well as areas where more resources (people/time/$) than are necessary are being put towards a SDoH issue. If nothing else it can provide knowledge of a group's existence so if there are other groups working on the same problem better communication or maybe even combining groups/expertise can happen.
